If you have an insurance policy that covers my services and you wish to utilize it to cover a portion of my fee, I will assist you in completing the forms required by your insurance company. Please be aware that it is your responsibility to secure preauthorization, and to know the limits of your coverage including “out-of-network” benefits, deductibles, limits on yearly payments, or number of sessions covered.

Cancellations: Your appointment is held exclusively for you. If you are unable to keep it for any reason, you must give at least 24 hours notice; otherwise you will be responsible for payment for the session. Insurance will not pay for missed appointments.
